Front of the Title (Buyer's Responsibilities):
Below the section labeled "Assignment of title," print the buyer's name and full address, including street address, city, state, and zip code.
On the subsequent line, record the vehicle's current mileage.
Under the heading "Signature(s) of Seller(s)," the seller(s) should sign their name(s). If two individuals are releasing ownership, both signatures must be present on the same line.
Following the signatures, the seller(s) should print their name(s). Adjacent to their printed names, indicate the date the vehicle was sold.
Proceeding, under the section "Signature(s) of buyer(s)," the buyer(s) should print their name(s). If there are two buyers, both signatures must fit on the same line. Directly next to their printed names, the buyer(s) should indicate the date of purchase.
